We will ask you to fill out an application and request medical records. 

We know that transitioning to a new assisted living facility is not easy. We want to do our best to meet your needs and allow you to age in place in our community. It is vital to learn as much as we can about you and your loved one by working with your medical professionals to obtain records associated with your physical and mental well being. 

We may also schedule a brief Nursing Assessment during this time. Our wonderful Nurse has extensive experience in geriatric care and will help guide you in making the best care decisions. 

As a licensed facility, we are required to ensure appropriate placement of all our residents. We will do this by reviewing the records provided and consulting with our Nurse. 

A Residency Agreement will be sent to you which details the terms and conditions of your placement in our community. If a comprehensive nursing assessment hasn't been completed yet, one will be scheduled within 14 days of move-in.

We will schedule your move in. When you're ready, we'll even help coordinate your transition, from home health or hospice, to pharmacy needs, visiting providers, we'll do everything we can to make the transition is as smooth as possible and you're settled in and happy. Within 2 weeks of move in, we'll work together to develop an individualized care plan to meet the needs of your needs.
